These Terms & Conditions govern your use of the services provided by Cognitive Trading & Investment Pty Ltd, trading as Valetudo Fitness and Physio ("we", "us", "our"). By booking an appointment or using our services, you agree to be bound by these terms.
We provide fitness and physiotherapy services including assessment, treatment, exercise prescription, and related health services. Our services are provided by qualified practitioners and are intended to support your health and wellbeing.
Booking: Appointments can be made online through our Cliniko booking system, by phone, or in person.
Confirmation: You will receive confirmation of your appointment via email or SMS. Please ensure your contact details are accurate.
Punctuality: Please arrive on time for your appointment. Late arrivals may result in reduced treatment time, and the full fee will still apply.
We require a minimum of 24 hours' notice for cancellation or rescheduling of appointments.
Cancellations made with less than 24 hours' notice, or failure to attend a scheduled appointment without notice, may incur a cancellation fee of up to 100% of the scheduled service fee.
We understand that emergencies happen. Please contact us as soon as possible if you are unable to attend, and we will do our best to accommodate you.
Fees: Our current fees are available on request or displayed at our premises. We reserve the right to change our fees at any time.
Payment: Payment is required at the time of your appointment unless otherwise arranged. We accept cash, credit card, debit card, and EFTPOS.
Health Fund Claims: If you have private health insurance, you may be able to claim a rebate for eligible services. We offer on-the-spot health fund claiming through HICAPS where available. Please bring your health fund card to your appointment.
Medicare: Some services may be eligible for Medicare rebates under certain referral arrangements. Please speak with us about your eligibility.
By attending an appointment, you consent to the assessment and treatment proposed by your practitioner. Your practitioner will explain the nature of any treatment and obtain your informed consent before proceeding.
You agree to provide accurate and complete information about your health history, current conditions, and any medications you are taking. Failure to disclose relevant information may affect the safety and effectiveness of your treatment.
You are responsible for:
Attending appointments on time
Providing accurate health information
Following treatment plans and home exercise programs as recommended
Informing us of any changes to your health or contact details
Treating our staff and other clients with respect
While we strive to provide safe and effective treatment, all health services carry some degree of risk. Your practitioner will discuss any specific risks associated with your treatment.
Results vary between individuals, and we cannot guarantee specific outcomes. Our services are not a substitute for emergency medical care or advice from your doctor.
We handle your personal information in accordance with our Privacy Policy, which is available on our website. By using our services, you consent to the collection, use, and disclosure of your information as described in our Privacy Policy.
All content on our website, including text, images, logos, and materials, is the property of Valetudo Fitness and Physio and is protected by copyright. You may not reproduce, distribute, or use our content without written permission.
To the maximum extent permitted by law, we exclude all liability for any loss, damage, or injury arising from or in connection with your use of our services, except where such liability cannot be excluded under Australian Consumer Law.

We reserve the right to refuse or discontinue services to any client at our discretion, including for non-payment, abusive behaviour, or failure to comply with these terms.
We may update these Terms & Conditions from time to time. Any changes will be posted on our website. Continued use of our services after changes are posted constitutes acceptance of the updated terms.
These Terms & Conditions are governed by the laws of Western Australia. Any disputes will be subject to the exclusive jurisdiction of the courts of Western Australia.
